Free online portal will help patients manage medical information in one location

Bay City, Mich.— McLaren Bay Region and McLaren Bay Special Care today launched My McLaren Chart, a secure online portal where patients can track and access their medical information in one central location, hospital officials announced today.

“My McLaren Chart will help us connect with our patients in a new and meaningful way by empowering our patients with more access to their medical information,” said Alice Gerard, President/CEO, McLaren Bay Region. “My McLaren Chart ensures the entire continuum of care — McLaren hospitals, community-based physicians, and the patients themselves — are sharing information and partnering together to manage the patient’s health.”

"Implementation of the Patient Portal is one more avenue to allow patients access to their own medical information at their fingertips,’ said Cherri Burzynski, President, McLaren Bay Special Care. “They will be able to electronically access their own medical information from a secure Internet connection worldwide and share with other practitioners as needed. This improves continuity of care globally and allows patient independence."

My McLaren Chart will provide patients with secure access to their medical information 24 hours a day, seven days a week; allow them to review clinical summaries and care instructions if they’ve had a recent visit to the hospital; give them lab and test results; and provide one central online location for patients to enter and manage other medical information. My McLaren Chart is free to patients and other health care providers.

My McLaren Chart goes live June 30, 2014. Once a patient enrolls in My McLaren Chart, it will be automatically populated with information from inpatient visits to McLaren hospitals that occur on or after June 30, 2014.

Information that is automatically added to My McLaren Chart includes lab and test results, clinical summaries and plans of care. In addition, patients may choose to enter their own data — such as medications and immunizations — to track all their medical information in one place. More features and functionality will be added to My McLaren Chart in the future.

All McLaren hospitals participate in My McLaren Chart. McLaren’s primary care, specialty physician offices, and free-standing diagnostic centers will begin sharing information on My McLaren Chart in the near future. Other independent physician practices and facilities are invited to join My McLaren Chart at no cost. A physician and/or practice must choose to enroll in My McLaren Chart to connect with their patients through the portal.
