Local Collaborative Studies Health Status of Saginaw and Undertakes Planning to Positively Impact Community Health

The Saginaw County Community Health Improvement Plan (CHIP) Partners formed in 2009, under the auspices of Alignment Saginaw, with a mission of developing a health improvement plan that would support and maintain a healthier Saginaw. Much work has occurred since we started in 2009.

We are pleased to present the 2017-2022 Saginaw County Roadmap to Health - Saginaw County Community Health Needs Assessment and Improvement. The Saginaw County Roadmap to Health outlines the goals, strategies, and activities related to the top health indicators identified from the most recent Community Health Needs Assessment, including input gathered from multiple data sources and at various interactive public meetings throughout Saginaw County conducted from September 2016 – January 2017. The July 13, 2017 press conference provides an opportunity to learn about local health related issues and ideas, as well as the accomplishments the CHIP Partners have made since 2009, with members of Saginaw County Community Health Improvement Plan (CHIP) Partners.

The Saginaw County Community Health Improvement Plan (CHIP) Partners includes representatives from the Saginaw Community Foundation, Saginaw County Department of Public Health, St. Mary’s of Michigan, Covenant HealthCare, Great Lakes Bay Health Centers, Saginaw Intermediate School District, Saginaw County Community Mental Health Authority, Saginaw County Department of Health and Human Services, the United Way of Saginaw, and Alignment Saginaw. This collaborative group encourages participation and input from organizations and individuals in its efforts to understand the health needs of our community, and to take collective actions aimed and fostering health and wellness for all.
